After coming from the hospital, efficient care is as important as when you are in the hospital. This is the time when your body recovers. During this time, the care given helps your body heal and go back to its normal state. This is temporary care, which is often called reablement or intermediary care.

Patients receive it for at least one or two weeks, which mainly depends on the condition. The recovery timespan primarily depends on how fast you and your body are able to cope with staying at home. The reablement care is arranged by the hospital staff before you leave. Talk to the discharge coordinator to make sure that this happens seamlessly.

When Do You Need Short-Term Care?

There are a number of instances when you might need intermediary care:

  • Post leaving the hospital- As mentioned earlier, you would need aftercare from professionals after getting treated by a hospital due to any injury or illness. This would help your body to heal properly.
  • Post injury or illness- If you have received an injury or fallen sick, reablement care can help you avoid going to the hospital. If you or anyone you know have fallen sick or got injured, contact your social services. They would arrange everything that you might need.
  • If you need help- Sometimes your health condition does not permit you to do everyday chores and tasks. For example, you might not be able to go down the stairs or bend down easily. In those cases, these types of care can support you and help you live your daily life with ease.

What Are The Different Types Of Aftercare You Can Get?

A team of people from the social services and NHS can help with these aftercare procedures and help you stay independent in your own home. Things involved in the care process might include doing daily chores like getting dressed or going down the stairs. They would start by offering you care but then would assist you to do all these things on your own. Depending on your health conditions, the team helping you might include a:

  • Nurse
  • Physiotherapist
  • Occupational Therapist
  • Doctors
  • Social Workers
  • Carers

Firstly, it will be decided what care you want and depending on that, and the plan would be formulated. Then, you would have to agree with the plan to start with the care procedure.

What Did Will happen After The Intermediary Care Finishes?

Once it is done, you, your family and the care providing team will decide on what happens next. For example, you might need some other type of care or help or continue with the same services if needed.

You should also consult with them about what you can do if anything goes wrong. Also, take assistance with different types of equipment that might help with your situation.
